This consent explains telehealth services accessed through Mediclab and helps you make an informed decision before you begin. Please read it carefully. By proceeding with an intake or treatment, you acknowledge that you have read and agree to this consent.
What telehealth is
Telehealth is the delivery of healthcare using secure technology — such as online questionnaires, secure messaging, phone, or video — instead of an in-person visit. A licensed healthcare provider reviews your information and, if clinically appropriate, may recommend or prescribe treatment and arrange pharmacy fulfillment.
What to expect
- You complete a health intake with accurate, complete information.
- A licensed provider reviews your information and may follow up for clarification.
- If appropriate, the provider issues a treatment plan and/or prescription; a pharmacy may dispense and ship medication.
- You receive follow-up and the ability to message your care team.

Convenient access to licensed providers from home, time savings, discreet care, and ongoing follow-up support.
Risks & limitations
- A provider may determine that telehealth is not appropriate for you and that an in-person evaluation, lab work, or referral is needed.
- Without an in-person exam, there is a possibility that a condition could go undetected.
- Technology may fail or be delayed; rarely, security protections could be breached despite safeguards.
- As with any treatment, medications carry possible side effects, which your provider will discuss.
No guarantee of treatment or outcome
Completing an intake or paying for a consultation or plan does not guarantee that any medication will be prescribed, dispensed, or shipped. Treatment decisions are made solely by the licensed provider based on clinical judgment and applicable law. Individual results vary and are not guaranteed.
Alternatives
You may choose in-person care, a different provider, or no treatment at all. You may ask questions at any time before deciding.
Privacy & security
Your information is handled in accordance with our Notice of Privacy Practices and Privacy Policy. Please use a private device and connection during your visit.
Your rights
- You may withhold or withdraw this consent at any time without affecting your right to future care.
- You may request information about your treatment and your records.
Fees
Fees for consultations, plans, and medications are shown before you pay. Payment for a consultation or membership does not guarantee a prescription.
