Notice of Privacy Practices
This Notice is provided on behalf of the independent licensed providers and professional medical entities that deliver care through the Mediclab platform (together, “we”). Mediclab provides technology and administrative support and handles protected health information (“PHI”) as a business associate of these entities under HIPAA.
How we may use and disclose your PHI
- Treatment — to provide and coordinate your care, including with providers and pharmacies.
- Payment — to process payments and, where applicable, billing.
- Health care operations — for quality, training, and administration.
- Business associates — with vendors who support our services under written agreements.
- As required by law — including public health, safety, and legal requirements.
Other uses and disclosures not described here will be made only with your written authorization, which you may revoke.
Your rights
- Inspect and obtain a copy of your records
- Request an amendment to your records
- Request an accounting of certain disclosures
- Request restrictions on certain uses and disclosures
- Request confidential communications (e.g., a specific contact method)
- Receive a paper or electronic copy of this Notice
- Be notified following a breach of unsecured PHI
Our duties
We are required by law to maintain the privacy of your PHI, provide this Notice, and abide by its terms. We may change this Notice and apply the changes to information we already hold; the current version will be posted here with its effective date.

If you believe your privacy rights have been violated, you may contact us at hello@mediclab.us or file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
